Admissions criteria
Our home health and hospice services are designed to provide care that is centered on the unique needs of our patients and their families. The first step in receiving services from Heritage Home Health & Hospice is ensuring that your loved one meets the criteria below.
Home Health Requirements
- Must be an overall effort to leave home
- Must have a physician's order
- Must have a skilled need
Hospice Requirements
- Doctor’s diagnosis of being terminally ill, meaning you have a life expectancy of 6 months or less
- Patient is no longer seeking aggressive treatment, but seeking end-of-life comfort care
Eligible diagnosis
Home Health services are focused on providing physician-ordered care in the home for patients who need temporary assistance following a medical event or procedure.

- COPD (Emphysema, Pulmonary Disease)
- CVA (stroke)
- After care following surgery
- Joint replacement
- Hypertension
- Diabetes
- Wound Care
Hospice services are focused on providing end-of-life care for patients, focusing on the physical and emotional well-being of the patient as well as their family. Those with one of the below illnesses who have received a doctor’s prognosis of 6 months or less are eligible to receive hospice care.
